David Dewayne Perkins of Whitley City, Kentucky, departed this life at his home on October 12, 2014 being 51 years, 10 months, and 3 days old. He was born in Muncie, Indianan, on December 9, 1962 the son of Doris Lee Perkins.
He worked as a Truck Driver for Leslies Tires; he was of the Baptist faith.
He is preceded in death by his wife Lisa May Young Perkins, Granddaughter: Shaelynn Nikol Crabtree, Grandmother: Julia Hutter, and Grandfather; Henry Perkins.
